A group of men (n=96) who had been diagnosed as infertile for at least 18 months were given the following nutritional formulation daily for four months: L-carnitine, acetyl-L-carnitine, fructose, citric acid, selenium, coenzyme Q10, zinc, vitamin C, vitamin B12 and folic acid (see abstract for exact dosages).  At the end of the study, sperm motility improved and 16 of the patients had achieved pregnancy.  The authors concluded that carnitine may be the key component of the supplement cocktail for improving sperm quality. Vitamin D helps leg ulcers heal
In this double-blind, placebo controlled trial, 26 patients Vitamins, Vitamin Dwith leg ulcers were given either placebo or 50,000 IU vitamin D weekly for two months.  Leg ulcer size, blood levels of vitamin D and pain was measured before and after the two month trial.  In the vitamin D group, leg ulcers were reduced in size by 28% while the placebo group had only a 9% reduction in ulcer size. The authors stated “there was a trend toward better healing in those with vitamin D reposition.” (Journal of Brazilian College of Surgeons, October 2012)

Omega 3sCLINICAL UPDATE - Omega 3s Can Replace Ibuprofen for PMS Pain
In this trial, a group of women were given either placebo or omega3 fatty acids for three months, followed by three months of the opposite regimen.  Symptoms of PMS, especially pain, were monitored. After 3 months of treatment, women who took the omega 3 capsules every day decreased their ibuprofen usage by 19% (first group) and by 53% (second group). AsthmaticsCLINICAL UPDATE - Carnitine Supplements Improve Lung Function in Asthmatics
A recent placebo-controlled study compared 50 children with asthma to 50 healthy controls and found that not only were levels of the amino acid carnitine lower in those with asthma, but supplementation with 350mg of L-carnitine daily for six months significantly improved lung function is those with asthma.   Researchers noted that carnitine’s primary role in transporting fatty acids into the mitochondria may explain its benefit to those with asthma – carnitine also transports fatty acids to lung membrane surfaces ensuring they are lubricated, explaining a mechanism by which carnitine improves pulmonary function. DiabetesCLINICAL UPDATE - Vitamin C Benefits Diabetics on Metformin
Seventy patients with type II diabetes were given either placebo or 500mg vitamin C twice a day.  All patients were taking metformin as well. After twelve weeks, measures of blood sugar control (fasting blood sugar, post-meal blood glucose and HbA1c levels) were all improved in the vitamin C group only. FertilityCLINICAL UPDATE - Vitamin E Increases Sperm Count
106 infertile men were included in this trial  – 64 were infertile due to low sperm count and 42 were infertile due to poor sperm motility.  They were divided into two groups – both groups received pharmaceutical therapy for fertility but only one group was simultaneously treated with a natural form of vitamin E.  Sperm count and motility improved more dramatically in the vitamin E treated group.  In fact, only five natural pregnancies occurred in the non-vitamin E group, while 15 natural pregnancies occurred in the vitamin E treated group.  In an unrelated trial, vitamin E was co-administered with fertility drugs to a group of women and compared to fertility drug treated women without vitamin E.  Although those treated with vitamin E did not have statistically higher rates of pregnancy, the women given vitamin E did show significantly improved endometrial thickness, which would facilitate future pregnancy. Muscle MassCLINICAL UPDATE - A New Role for Omega 3s in Building Muscle Mass
A group of women (average age = 64 years old) were put on a strength training program for 90 days.  Some of the women were given fish oil supplements while others received no supplement. Measures of muscle strength were taken and those taking fish oil showed significantly higher muscle development.  Researchers hypothesized that this increase in muscle development during strength training is due to the role omega 3 fatty acids play in plasma membranes and muscle cell function. (American Journal of Clinical Nutrition, February 2012)

SerineCLINICAL UPDATE - High Dose Serine Improves Neuropathy: Case Report
One of the most common inherited neurological disorders, Charcot-Marie-Tooth disease affects approximately 1 in every 2500 people and is characterized by muscle wasting and peripheral neuropathy. In this case report, a male patient with polyneuropathy had a serine synthesis defect.  Deficiency in the amino acid serine often manifests neurologically and in this case, high-dose supplementation of serine improved his neuropathy symptoms. A New Twist on a Common Supplement: Using Omega 3’s to Lower Homocysteine

Homocysteine continues to gain acceptance as a risk factor in a number of conditions.  Once thought to be associated with mostly cardiovascular disease, homocysteine is now recognized as a contributor to numerous states of dysfunction, including arthritis, cognitive decline, osteoporosis and many more.  Given the vast reaching effects of homocysteine, having an ideal treatment protocol in place to address elevations seems necessary, if not even absolutely crucial.

Examination of the literature to lower homocysteine can be confusing at times, with some studies showing lack of efficacy of B vitamins.  However, these studies were not without criticism and ultimately a return to the basics of physiology has shown B vitamins to be an efficacious intervention.  When looking at the methionine cycle and delving deeper into the biochemistry of homocysteine, it becomes rather clear that without methyl donors, B Vitaminshomocysteine conversion to methionine gets stalled and homocysteine begins to build up.  As such, the methyl donors B12 and folate have become the primary interventions for homocysteine lowering therapy.

For some however, B12 and folate therapy are not enough.  Most of these individuals fall under the category of having a variant for the methyltetrahydrofolate reductase enzyme, commonly abbreviated MTHFR.  Variants in this enzyme impair the ability to reduce folate, thus making the conversion of homocysteine to methionine inefficient and promoting accumulation.  This is seemingly overcome fairly easily in elevated cases by the use of L 5-methyltetrahydrofolate.  Upon normalization of homocysteine in this manner, levels can generally be kept in an ideal range by the use of folate, a non-synthetic, as compared to folic acid.

As relatively straightforward as this all seems, the story does not end here.  A deeper look at the influences on our genes shows the complexity of modulating homocysteine.  There must exist a constant balance between homocysteine and methionine.  If the objective is to lower homocysteine, downregulating the enzymes that lead to its conversion and upregulating those that convert it to methionine would appear ideal.  To do this very thing, one should look no further than omega 3’s.  The polyunsaturated omega 3’s, known to most of us as fish oils, canOmega 3s directly affect the expression of the genes that control the enzymes linked to homocysteine metabolism.  Since the omega 3’s do not directly insert into the biochemical pathway of the methionine cycle, their actions clearly have to be based on another mechanism of action, one we now recognize as genetic.

Consider the therapeutic potential here for a number of patients.  Let’s take the aforementioned conditions for which homocysteine is known to affect.  Equally validated in the literature to support cardiovascular disease, arthritis, cognitive decline, and osteoporosis, omega 3’s will not only reduce inflammation but will also modify genetic expression of those enzymes metabolizing homocysteine.  This makes testing and adding omega 3’s to any clinical protocol at minimum a consideration given the considerable effects of homocysteine and the therapeutic potential of omega 3’s.

Nutritional Considerations of ADHD & Autism

Recent years has seen an unprecedented rise in autism and attention deficit hyperactivity disorder (ADHD).

Although researchers speculate on the reason for this rise, many factors likely contribute, including more accurate diagnosis. However, overwhelming evidence suggests that nutritional deficiencies may be a contributing factor.

OMEGA-3 FATTY ACIDS ARE KEY
Our brain and nerves are composed mostly of fat. The most important of these are called omega-3 fatty acids and are found primarily in fish or fish oil supplements. Also called EPA and DHA, they are absolutely necessary for human health, and their concentration in the brain makes them key players in neurological disorders such as autism and ADHD. Brain and nerve growth throughout childhood is extraordinarily rapid, and the need for omega-3 fatty acids remains critical all the way through adolescence and into adulthood. Overwhelming evidence implicates deficiencies in omega-3 fatty acids for the rise in autism and ADHD. Research shows that children with low scores on behavioral assessment tests consistently have lower omega-3 fatty acids levels, and when supplemented with fish oils, the symptoms of ADHD in these children such as hyperactivity, impulsiveness, and inability to pay attention - dramatically improve.

Omega 3 & 6WHY MEASURE THE OMEGA-6 TO OMEGA-3 RATIO?
We are familiar with the expression that a child’s brain is like a sponge, ready to absorb information at an astounding rate. A truly appropriate analogy, it stems from the fact that our brains can actually create nerve pathways in response to new experiences and learning environments. Called “neuronal plasticity,” this phenomena is crucial for long-term memory and learning. Proper levels of the omega-3 fatty acid, DHA (docosahexaenoic acid), is important for membrane fluidity. The ratio of omega-6 fatty acids, which differ in structure and function, to omega-3 fatty acids affect neuronal plasticity as well. Scientists now agree that the ratio of omega-6 fats to omega-3 fats is as important as the actual levels, especially in autism and ADHD. A lower ratio is better and when this ratio is improved, symptoms of autism and ADHD can improve as well.

ZINC – THE MOOD MINERAL
Zinc deficiency is often implicated in ADHD because it is an important co-factor to several neurotransmitters, which directly affect mood and learning ability. Specifically, zinc affects the production of dopamine, a feel-good chemical in our brains that is needed for learning and certain emotions like motivation and pleasure. In fact, studies show that zinc levels correlate with anxiety and behavioral problems, as well as have a significant effect on information processing in boys with ADHD. Since zinc levels are much lower in autistic and ADHD individuals, children with ADHD show positive behavioral and cognitive results after zinc supplementation.In addition, zinc is essential for proper elimination of the toxic metal mercury from our brain tissue, which has also been linked to autism and ADHD.

Vitamins & AutismVITAMINS AND AUTISM
Low levels of vitamin D have been linked with autism and in some cases of severe deficiency, high-dose vitamin D therapy actually reversed some of the autistic behaviors. Some research even suggests that the nutritional status of the mother during gestation can affect behavior in children. One study confirmed that low folate status in pregnancy was associated with hyperactivity in children. Other studies show that persons who carried a common gene that predisposes them to folate and vitamin B12 deficiency (called the MTHFR gene) were more likely to suffer from ADHD. Supplementation with thiamine (vitamin B1) has shown clinical benefit to some autistic children. Specifically, a deficiency in vitamin B1 has been associated with delayed language development in childhood.When deficient, biotin (vitamin B7) can potentially cause neurological problems associated with autism since the brain is quite vulnerable to biotin deficiency.

MAGNESIUM AND VITAMIN B6 – A WINNING COMBINATION
Like most nutrients, magnesium and vitamin B6 work together in improving clinical symptoms of autism and ADHD.

When a group of autistic children were supplemented with magnesium and vitamin B6, 70% of the children showed improvement in social interaction and communication. Interestingly, when the supplements were stopped, the clinical symptoms reappeared. In another study, physical aggression and inattention improved after supplementation with magnesium and vitamin B6 for a few months.

NeurotransmittersTHE ROLE OF NEUROTRANSMITTERS
Neurotransmitters are tiny chemicals that transmit information from the outside world to various parts of our brains and from our brains to the rest of our bodies. Although neurotransmitters, such as choline, glutamine, asparagine and inositol may not be recognized as household names, they profoundly affect emotions, thinking and social behavior. For example, levels of glutamine and asparagine are lower in autistic children and some adults with ADHD.

AN AMINO ACID THAT IMPROVES CARNITINE – BEHAVIOR
Carnitine is an amino acid whose primary function is to transport fatty acids, including the ever-so-important omega-3 fatty acids into cells so they can be used for energy. In autistic individuals, carnitine levels are significantly reduced, which then affects the patient’s ability to use the fatty acids that are so critical to their learning and social development.
A recent study demonstrated that carnitine can reduce hyperactivity and improve social behavior in boys diagnosed with ADHD, and may actually represent a safe alternative to the use of stimulant drugs for the treatment of ADHD in children.

ADHD AND AUTISM – AN OXIDATIVE STRESS DISORDER?
Oxidative stress is a term used to describe damage to our cells that occurs on a daily basis throughout our bodies. Using Micronutrient Testing to Improve the Management of Autoimmune Conditions

Autoimmune Disorders and MNTAutoimmune conditions are a rapidly growing segment of the medical population. They go by such names as Hashimoto’s thyroiditis, Rheumatoid Arthritis, Ulcerative Colitis and Crohn’s Disease just to name a few. The noted growth of these conditions appears to be related to many factors, including increased stressors, poor diet, and degradation of the lining of the gastrointestinal tract allowing formation of a hyperpermeable gut.

Management of these conditions is not always a straightforward process and most therapies have traditionally centered on reducing inflammation. However, long term management of autoimmune conditions requires not only reducing the total inflammatory burden, but also working to add balance to the immune system. This is dependent on the function of the cells of the immune system. Therefore, having an assessment method that directly targets the cells of the immune system would serve as an invaluable tool in the management of autoimmune conditions.

One of the hallmarks of autoimmune conditions is that they promote the destruction of a large amount of tissue secondary to inflammation. Typically when this occurs, the ability of the cells to resist further destruction is reduced, leaving them even more susceptible. Autoimmune diseases are noted for a lack of regulation of the immune system. During states of autoimmunity, the T lymphocytes known specifically as T regulatory cells lose their ability to guide the appropriate type of immune response. The misguidance of the ideal immune response is what eventually manifests as the fulminant inflammation associated with autoimmune diseases. Proper function of the T regulatory cells is dependent on vitamin D, glutathione and omega 3’s. A deficiency in any of these nutrients opens the door to immune system dysfunction and increased autoimmunity. Lower levels of glutathione and vitamin D appear to be most representative of the state of the T regulatory cells and the likelihood of dysfunction. When these levels are low, especially glutathione, it can be noted that the inflammatory burden has become overwhelming. Ironically, the micronutrient test is probably one of the better suited tests to pick up the needed repletion of these nutrients since it is looking directly at the T lymphocytes.

An area that appears to open the door to autoimmunity is a breakdown in the integrity of the gastrointestinal lining, also referred to as a hyperpermeable gut. Nutrients such as glutamine, vitamin A and zinc are needed to maintain the integrity of these cells. As these nutrients trend lower, the suspicion of the gastrointestinal tract as a player in the autoimmunity milieu grows.

While not the only test warranted in managing autoimmunity, micronutrient testing should be one of the first considerations as it provides a window into not only areas that contribute to the autoimmune process, but also details the specific nutrients needed to manage it.

Why Do Omega-3 Fatty Acids Affect So Many Functions In Our Body?

Omega 3 Fatty AcidsThe answer is simple: cell membrane flexibility.  Every cell has a cell membrane.  When this cell membrane is rigid, it does not work well.  When it is flexible, the chemicals that run our bodies - hormones, proteins, enzymes, vitamins, minerals, fats, etc - can move in and out of cells efficiently as needed, thus making the cells healthier, since the materials they need to function well are available.  When the cells work well, the tissues that are made of cells work well.  When tissues work well, the whole system works well and ultimately leads to overall improved health of the entire person.

For example, when there are enough omega-3 fatty acids available through either diet or supplementation, they will be absorbed into cells in the heart, making their cell membranes flexible, but strong.  Consequently, the heart and arteries are stronger and therefore the entire cardiovascular system benefits.  In fact, in the same way that omega-3 fats make cell membranes more flexible, the dangerous trans fats do the exact opposite - they are absorbed into the cell membranes making them stiff and unable to do their job.  Just as stiff joints or stiff arteries are unhealthy, so are inflexible cell membranes.  And since cell membranes are an integral part of every tissue in the body, the level of omega-3 fatty acids a person has can affect just about everything (see below).
