Is carnitine the answer for male infertility?male, infertility
A group of men (n=96) who had been diagnosed as infertile for at least 18 months were given the following nutritional formulation daily for four months: L-carnitine, acetyl-L-carnitine, fructose, citric acid, selenium, coenzyme Q10, zinc, vitamin C, vitamin B12 and folic acid (see abstract for exact dosages).  At the end of the study, sperm motility improved and 16 of the patients had achieved pregnancy.  The authors concluded that carnitine may be the key component of the supplement cocktail for improving sperm quality. (Italian Archives of Urology and Andrology, September 2012)

Vitamin D helps leg ulcers heal
In this double-blind, placebo controlled trial, 26 patients Vitamins, Vitamin Dwith leg ulcers were given either placebo or 50,000 IU vitamin D weekly for two months.  Leg ulcer size, blood levels of vitamin D and pain was measured before and after the two month trial.  In the vitamin D group, leg ulcers were reduced in size by 28% while the placebo group had only a 9% reduction in ulcer size. The authors stated “there was a trend toward better healing in those with vitamin D reposition.” (Journal of Brazilian College of Surgeons, October 2012)

Complexity of methylation reactions gains insightmethyl donor, nutrients
This review emphasizes how methyl donor nutrients such as choline, folic acid and methionine interact and how consumption (via supplement or food) of one can have sparing effect s on another – such as choline’s  sparing effect on methionine, for example. The Importance of Vitamins

New study says multivitamins just don’t cut it when it comes to preventing heart diseaseVitamins, multi-vitamins

In the landmark Physician’s Health Study II, authors concluded that taking a multivitamin for over a decade did nothing to prevent cardiovascular events such as myocardial infarction or stroke. The study monitored 14,641 male doctors for over eleven years who took either a daily multivitamin or placebo and no differences in cardiovascular events or mortality was found between the two groups.  Since evidence linking deficiencies to heart disease is strong (see vitamin D study below on 45,000 patients), some conclude that a multivitamin is simply not effective in correcting deficiencies and that targeted supplementation for the individual is a better approach. Vitamin C reduces fatigue and perception of effort after exercise

Vitamins, vitamin cIn this interesting study on twenty obese adults, each were given either 500 mg of vitamin C or placebo daily for four weeks.  Their diet was strictly controlled for vitamin C content and their heart rates and fatigue scores as well as subjective perceptions of exertion were measured after exercise.  Those taking vitamin C had lower fatigue scores and those on placebo had higher fatigue scores. Heart rates and “ratings of perceived exertion” were also improved in the vitamin C group. (Nutrition, January 2013)

CLINICAL UPDATE – Nutritional status of mom has major implications foNutrition, Vitamins, vitamin B12r baby 

Three recent studies emphasize the fallacy of the paradigm “baby gets what he needs at the expense of the mother.” In one study done in the Netherlands, researchers evaluated over 3200 mothers for blood folate levels and their children at three years of age for behavioural and emotional problems. Although the implications of low folate status of the mother on neurological development is well established, this study reported that low folate status of the mother is linked with a “higher risk of emotional problems in the offspring.”  An unrelated study in India found a similar result with vitamin B12 and heart function. Cord blood of mothers was compared to the cardiac function of their babies. “Children born to mothers with a lower vitamin B12 status have a reduced cardiac sympathetic activity.”  Finally, a review of studies done between 1999 and 2011 concluded that vitamin D deficiency of mom is linked to gestational diabetes. CLINICAL UPDATE - E Zinc deficiency common in diabetics                                                        

Diabetes, E-zinc, serumSerum and intracellular levels were measured in 75 type I and II diabetics and compared to 75 age matched controls.  Zinc levels were significantly lower in diabetic patients.  Authors of the study reported that in vivo and in vitro studies indicate that zinc promotes insulin signalling and supplementation may be a potential treatment in zinc-deficient diabetics. (Journal of Nutritional Biochemistry, November 2012)

CLINICAL UPDATE – N-acetylcysteine lowers irritability in autistic kids

In this randomized clinical trial on 33 autistic children ages three to ten years old, a dose of 900mg N-acetylcysteine was given twice daily for 12 weeks.   At each 4 week interval, a standardized test was given to measure irritability and behaviour on each child.  After three months, those receiving the high-dose N-acetylcysteine had significant improvements on their irritability compared to the placebo group. (Biological Psychiatry, June 2012)

CLINICAL UPDATE – Vitamin D trial shows it can reduce body fat

In a double-blind, randomized, placebo-controlled trial, 77 people were given either 25mμg of vitamin D or placebo for 12 weeks. Researchers concluded that “supplementation with vitamin D3 caused a statistically significant decrease in body fat mass.”  Specifically, the vitamin D group lost six pounds while the placebo group lost an average of only one pound. CLINICAL UPDATE – Vitamin K reduces diabetes risk

diabetes, vitaminsDietary intake of phylloquinone (a form of vitamin K) was assessed in over a thousand men and women.  Those with increased intake of vitamin K hahave lower rates of diabetes.  As a follow up to the study, the people who increased their dietary vitamin K during the follow-up had 51% reduced risk of diabetes. CLINICAL UPDATE – Vitamin E kills cancer cells

Cancer Cells Immune SystemThe alpha, delta and gamma tocotrienol forms of vitamin E were evaluated on leukemic cancer cells.  The delta tocotrienol form of vitamin E, which was the most potent in killing cancer cells, changed the DNA of the cancerous cells in such a way that it induced the cancerous cells to undergo apoptosis (cell death). (Microscopy and Microanalysis, June 2012)

THE ROLE OF MICRONUTRIENTS IN DIABETIC HEALTH

Vitamin E - Confers protection against diabetes by protecting pancreatic B-cells from nutrition reference chart for diabetesoxidativestress induced damage; May prevent progression of type I diabetes.

Vitamin D - Lowers risk of type I and 2 diabetes; Suppresses inflammation of pancreatic B-cells. Vitamin D receptor gene linked to diabetes.

Vitamin B3 - Preserves B-cell function in type I diabetics; Part of GTF (glucose tolerance factor) which facilitates insulin binding.

Vitamin B12 - Deficiency common in diabetics because metformin depletes B12.

Chromium - Helps insulin attach to cell's receptors increasing glucose uptake into cell; Deficiency can cause insulin resistance; Supplementation trials show dose-dependent benefits for type II diabetics.

Biotin - Stimulates glucose-induced insulin secretion in pancreatic B-cells; High dose biotin can improve glycemic control in diabetics.

Magnesium - Deficiency reduces insulin sensitivity; Low magnesium exacerbates foot ulcers in diabetics.

Zinc - Needed in the synthesis, storage and secretion of insulin; Protects pancreatic B-cells from damage; Affects the expression of genes linked to diabetes.

Lipoic Acid - Enhances glucose uptake in skeletal muscle tissue; Improves glucose tolerance in type 2 diabetics; very effective treatment for diabetic neuropathy.

Glutathione & Cysteine - Glutathione-containing enzymes protect B-cells which are particularly sensitive to oxidative stress; Type 2 diabetics have abnormal antioxidant status; Supplementation with the glutathione precursor cysteine restores antioxidant status.

Coenzyme Q10 - Protects kidney from diabetes related damage; Improves glycemic control in type 2 diabetics.

Glutamine - Stimulates a hormone called GLP-I (glucagon-like peptide I) that regulates insulin secretion after meals; Improves insulin signaling and sensitivity.

Carnitine - Reduces and even prevents pain from diabetic neuropathy; Improves insulin sensitivity by increasing glucose uptake and storage.

Inositol - Evidence suggests that inositol may be effective in treating diabetic neuropathy.

Vitamin C - Lowers glycolysated hemoglobin (HbA1c) and fasting and post-meal glucose levels and in type 2 diabetics.

Omega 3sCLINICAL UPDATE - Omega 3s Can Replace Ibuprofen for PMS Pain
In this trial, a group of women were given either placebo or omega3 fatty acids for three months, followed by three months of the opposite regimen.  Symptoms of PMS, especially pain, were monitored. After 3 months of treatment, women who took the omega 3 capsules every day decreased their ibuprofen usage by 19% (first group) and by 53% (second group). (International Journal of Gynaecology and Obstetrics, April 2012)

AsthmaticsCLINICAL UPDATE - Carnitine Supplements Improve Lung Function in Asthmatics
A recent placebo-controlled study compared 50 children with asthma to 50 healthy controls and found that not only were levels of the amino acid carnitine lower in those with asthma, but supplementation with 350mg of L-carnitine daily for six months significantly improved lung function is those with asthma.   Researchers noted that carnitine’s primary role in transporting fatty acids into the mitochondria may explain its benefit to those with asthma – carnitine also transports fatty acids to lung membrane surfaces ensuring they are lubricated, explaining a mechanism by which carnitine improves pulmonary function. (Journal of Allergy, 2012)

DiabetesCLINICAL UPDATE - Vitamin C Benefits Diabetics on Metformin
Seventy patients with type II diabetes were given either placebo or 500mg vitamin C twice a day.  All patients were taking metformin as well. After twelve weeks, measures of blood sugar control (fasting blood sugar, post-meal blood glucose and HbA1c levels) were all improved in the vitamin C group only. Brain InjuryCLINICAL UPDATE - Zinc: A New Role for Traumatic Brain Injury
In this review, the protective role of zinc against traumatic brain injury is elucidated.  The authors stat that human clinical data “suggest that zinc may increase resilience” and can be used “to improve cognitive and behavioural deficits associated with brain injury.” (Nutrition Reviews, July 2012)

FertilityCLINICAL UPDATE - Vitamin E Increases Sperm Count
106 infertile men were included in this trial  – 64 were infertile due to low sperm count and 42 were infertile due to poor sperm motility.  They were divided into two groups – both groups received pharmaceutical therapy for fertility but only one group was simultaneously treated with a natural form of vitamin E.  Sperm count and motility improved more dramatically in the vitamin E treated group.  In fact, only five natural pregnancies occurred in the non-vitamin E group, while 15 natural pregnancies occurred in the vitamin E treated group.  In an unrelated trial, vitamin E was co-administered with fertility drugs to a group of women and compared to fertility drug treated women without vitamin E.  Although those treated with vitamin E did not have statistically higher rates of pregnancy, the women given vitamin E did show significantly improved endometrial thickness, which would facilitate future pregnancy. Muscle MassCLINICAL UPDATE - A New Role for Omega 3s in Building Muscle Mass
A group of women (average age = 64 years old) were put on a strength training program for 90 days.  Some of the women were given fish oil supplements while others received no supplement. Measures of muscle strength were taken and those taking fish oil showed significantly higher muscle development.  Researchers hypothesized that this increase in muscle development during strength training is due to the role omega 3 fatty acids play in plasma membranes and muscle cell function. (American Journal of Clinical Nutrition, February 2012)

SerineCLINICAL UPDATE - High Dose Serine Improves Neuropathy: Case Report
One of the most common inherited neurological disorders, Charcot-Marie-Tooth disease affects approximately 1 in every 2500 people and is characterized by muscle wasting and peripheral neuropathy. In this case report, a male patient with polyneuropathy had a serine synthesis defect.  Deficiency in the amino acid serine often manifests neurologically and in this case, high-dose supplementation of serine improved his neuropathy symptoms. Nutritional Considerations of Pain

PainBelow is a list of various nutrients that affect a person affected with body pain.

  • Cysteine - reduces pain caused by systemic inflammation due to its potent antioxidant properties.
  • Inositol - in animal studies, treatment with inositol induces antinociception (pain reduction).
  • Oleic Acid - this fatty acid is a precursor of oleamide, an analgesic that affects neurotransmitters such as dopamine, serotonin, acetylcholine and GABA (gamma amino butyric acid), all of which play a role in pain signaling.
  • Carnitine - deficiency of this amino acid may manifest as muscle weakness, pain (myalgia) or neuropathy. Supplementation reduces several types of chronic pain.
  • Magnesium - lowers pain by blocking NMDA receptors in spinal cord; effective in reducing post-operative pain.
  • Minerals:
    • Manganese - a cofactor for the potent antioxidant superoxide dismutase, which fights free radicals, a known source of pain.
    • Copper - supplementation can relieve arthritic pain.
    • Selenium - treatment with this mineral improves muscle pain in deficient patients.
    • Zinc & Calcium - research suggests both play a role in the transmission of pain signals through nerves.
  • Choline - activates specific receptors in brain and spine that lower acute pain.
  • Vitamin B1, B2, B6, B12 - these produce a dose dependent decrease in various kinds of pain (heat, pressure, chemical); increases sensitivity to pain meds; their effect is likely mediated through serotonergic neurotransmitters.
  • Vitamin D - deficiency often presents clinically as muscle or bone pain.
  • Lipoic Acid - very effective treatment for neuropathic pain.
  • Antioxidants - clinical trials show antioxidant therapy is an effective treatment for chronic pain
    • Vitamin E - reduces neuropathic pain
    • Vitamin C - can lower morphine consumption after surgery
    • Coenzyme Q10 - relieves statin-induced myopathy.

The Importance of Nutrition on Weight Loss

Micronutrient TestingBelow is a list of various nutrients that affect a person's ability to gain or lose weight.

  • Zinc - reduces leptin, a beneficial hormone that regulates appetite, which is reversed by zinc repletion.
  • Asparagine - this amino acid increases insulin sensitivity which helps the body store energy in muscle instead of storing it as body fat.
  • Biotin - boosts metabolism by improving glycemic control (stabilizes blood sugar) and lowering insulin, a hormone that promotes fat formation.
  • Carnitine - carries fatty acids into cell so they can be burned for fuel; Helps reduce visceral adiposity (belly fat).
  • Calcium - inhibits the formation of fat cells; Also helps oxidize (burn) fat cells.
  • Lipoic Acid - improves glucose uptake into cells, which helps a person burn carbohydrates more efficiently.
  • Chromium - makes the body more sensitive to insulin, helping to reduce body fat and increase lean muscle.
  • Vitamin B5 - taking B5 lowers body weight by activating lipoprotein lipase, an enzyme that burns fat cells. One study linked B5 supplementation to less hunger when dieting.
  • Magnesium - low magnesium in cells impairs a person’s ability to use glucose for fuel, instead storing it as fat; Correcting a magnesium deficiency stimulates metabolism by increasing insulin sensitivity. Magnesium may also inhibit fat absorption.
  • Glutamine - reduces fat mass by improving glucose uptake into muscle.

VitaminsCLINICAL UPDATE - SELENIUM: KNOW IF YOU HAVE TOO MUCH OF A GOOD THING
An exhaustive review of the benefits selenium has on human health emphasizes that there is a definite “U-shaped link with status.”  The paper epitomizes the philosophy that more-is-not-always-better for vitamins and minerals.  Although low selenium status has been linked to several diseases – heart disease, infertility, low immunity, poor cognitive function, thyroid disease and cancer – the authors state that although “additional selenium intake may benefit individuals with low status, those with adequate status might be affected adversely and should not take selenium supplements.”  (Lancet, March 2012)

Heavy Metal ToxicityCLINICAL UPDATE - NUTRIENT DEFICIENCY CAUSING HEAVY METAL TOXICITY?
A case study on a 37 year old man suggests that multiple micronutrient deficiencies played a role in the cause of his multiple sclerosis.  The authors state he had several key nutrient deficiencies, which they think impaired his ability to excrete harmful metals.  They sate that “nutritional treatment may be an effective approach to this disease” due to the role of nutrients in various detoxification pathways. Blood PressureCLINICAL UPDATE - META-ANALYSIS FINDS VITAMIN C SUPPLEMENTS LOWER BLOOD PRESSURE
In a review of 29 randomized controlled trials of oral vitamin C, authors found that vitamin C supplements reduced both systolic and diastolic blood pressure.  The reduction in blood pressure was more pronounced in people with existing hypertension.  In another meta-analysis, magnesium supplementation also lowered blood pressure. (American Journal of Clinical Nutrition, May 2012; European Journal of Clinical Nutrition, April 2012)

Oxidative Stress and HypothyroidismCLINICAL UPDATE - FOLIC ACID LOWERS OXIDATIVE STRESS IN HYPOTHYROIDISM
In two separate studies, antioxidant function was evaluated after hypothyroidism was induced.  Since basal metabolic rate is lower in hypothyroism and most oxidative stress in the body stems from basic metabolic functions, some researchers hypothesize that hypothyroidism should result in lower levels of oxidative stress.  But in this recent animal study, the authors found higher levels of oxidative stress in the hypothyroid (vs normal thyroid) state, as well as higher levels of homocysteine.  Treatment with folic acid ameliorated these effects.  In another study, authors found that hypothyroidism reduced antioxidant function, although the serum levels of several antioxidants. ToxicityCLINICAL UPDATE - COQ10 PROTECTS LIVER FROM ACETAMINOPHEN TOXICITY
A single toxic dose of acetaminophen was administered in an animal study and liver damage was monitored.  After 1 hour and after 12 hours, an injection of coenzyme Q10 was given. The results showed that the coQ10 injections protected the liver from acetaminophen-induced damage.  The coQ10 also attenuated the loss of zinc and selenium that occurred after acetaminophen administration. (Environmental Toxicology and Pharmacology, March 2012)

Infertility and Vitamin ECLINICAL UPDATE - VITAMIN E MAY HELP INFERTILE WOMEN
103 women with unexplained infertility were divided into two groups – 50 women were given a drug to induce ovulation (clomiphene citrate) combined with 400IU per day of vitamin E and 53 women were induced to ovulate without the administration of vitamin E.  The supplemented group had a significantly thicker endometrium and authors suggest that vitamin E may be beneficial to women with unexplained infertility by improving the “endometrial response” and that it may “modulate the antiestrogenic effect of clomiphene citrate.” (Journal of Assisted Reproduction and Genetics, February 2012)

Carbohydrate MetabolismCLINICAL UPDATE - BIOTIN DEFICIENCY IMPAIRS CARBOHYDRATE METABOLISM
Researchers found that biotin deficiency negatively alters carbohydrate metabolism.  Specifically, biotin deficiency resulted in an impaired glucose and insulin tolerance test, suggesting “defects in insulin sensitivity,” according to the authors. (Journal of Nutritional Biochemistry, April 2012)
